To start your sleep apnea treatment, we will need the following:

1 - Diagnostic Sleep Study: Fax a copy of your full sleep study report to our office at 413-729-3646.

2 - Prescription Form: A completed prescription form or letter of medical necessity (PDF).

3 - Insurance Information: We will bill Medicare and most insurance providers for you. Please call us and we will check your coverage for you.
